**Tuition Amount Includes The Modern & Pilates Classes (for Levels 5, 6 and 7).

Jazz Class will be $20/Month Extra.

Level 4 Modern will be $20/month extra.

All students may wear dance sweaters. Sweatshirts or bulky sweaters are not allowed for Intro-7. Level 3 thru 7 students may wear waist to ankle leg warms and must wear black elastic belt around waist. Level 7 students may wear skirts, but only in center.

Please review the above information carefully and make any necessary corrections. Please sign this form/release and return it to the school with the first month's tuition and registration fees by August 15th to ensure enrollement in class. Your child is not considered registered unless this form is returned. Priority for acceptance in classes will be given based on when forms are received.

Students in Level 4 may register for the 4/5 Modern Class, which will cost an extra $20 per student.

Students in Levels 5, 6, and 7 may register for the Jazz Class, which will cost an extra $20.

The Dance Centre of Southwest Virginia will present "BEAUTY AND THE BEAST", an original ballet by School Director, Sandra Smeltzer, with additional choreography by faculty members Katharine Dorman, Pedro Szala and Abigail Williams

All Tickets Are $20 And Will Be Available At The Dance Centre Beginning April 21st.
